By signing below, you certify that the following statements are true and accurate:
- I am a U.S. citizen or lawfully admitted permanent resident (i.e., green card holder).
- This contribution is made from my own funds, and not those of another.
- This contribution is not made from the funds of a corporation, labor organization, or federal contractor.*
- This contribution is made on a personal credit card or debit card for which I have the legal obligation to pay, and is not made either on a corporate or business entity card or on the card of another person.*
- I am at least eighteen years old.
You may make contributions of up to $5,000 per calendar year to Mental Health PAC’s Contribution Account. Contributions (or any portion of contributions) from an individual that aggregate above that limit will be allocated to a separate bank account solely for contributions from individuals to our Non-Contribution Account, and contributions from PACs that exceed that limit will be allocated to our overall Non-Contribution Account.
