By submitting this form, I agree to the following:

• I am at least 18 years of age.

• I am a US citizen or lawfully admitted permanent resident (i.e., green card holder).

• This contribution is made from my own personal funds, and the funds are not being provided to me by another person or entity for the purpose of making a contribution.

• The committee may only accept contributions from individuals and business entities.  Entities controlled by an individual have a single total contribution limit of $750 across the controlling individual and the controlled entities.

• This contribution is not being made under a false name, is not being made under another person’s name, and has not been and will not be reimbursed. 

• The committee may only accept a maximum contribution of $750 per individual or entity.

• Spouses may write separate checks or write one check for a maximum of $1,500 if the check is signed by both spouses.

• The committee must collect your full name, address, occupation, and employer before depositing the contribution.

• Contributions are not tax deductible.

• The committee may not accept contributions from any individual who represents – or has represented within the past 12 months – an entity regarding a matter before the City of San Juan Capistrano Planning Commission.

• The committee may not accept contributions of more than $500 from any party with a pending matter before the Planning Commission, or from a party that received a final decision from the City of San Juan Capistrano Planning Commission in the previous 12 months.
