(1) Authority. I am duly authorized to make this contribution on behalf of the entity.

(2) Citizenship. The entity is organized under the laws of a U.S. state, is not a foreign corporation, and is not directed or controlled by a foreign national as defined in 52 U.S.C. § 30121 or Miss. Code Ann. § 23-15-819.

(3) Entity Funds. This contribution is made from the entity's own funds and not as a conduit for any other person or entity. The entity has not been reimbursed by, and is not making this contribution on behalf of, any individual or other entity.

(4) Not a Federal Contractor. The entity is not a federal contractor, nor is this contribution being made by or on behalf of a federal contractor.

(5) Judicial Nonpartisan Compliance. The entity is not a political party, nor a committee or political committee affiliated with a political party, and is not making this contribution on behalf of any political

party or party-affiliated committee. (Required by Miss. Code Ann. § 23-15-976.)

(6) Contribution Limit. I understand that Mississippi law limits corporations, incorporated companies, and incorporated associations to $1,000 in aggregate contributions per calendar year (Miss. Code Ann. §97-13-15). Per committee policy, limited liability companies (LLCs) are also treated as subject to this $1,000 calendar-year limit. This contribution, when added to any prior contributions to the Committee from this entity during the current calendar year, does not exceed $1,000.

(7) Truthfulness. All information I have provided in this form is true and complete to the best of my knowledge.

(8) Refund Acknowledgment. I understand that if any portion of this contribution is later determined to exceed the legal limit or to be otherwise non-compliant, the Committee may refund the excess or the entire contribution and report the contribution as required by law.
